The goal of the dental program is to provide quality dental care to associates at competitive rates.  Delta Dental provides the dental plan.  Rates are subject to change.

ELIGIBILITY:  Regular full-time associates.

EFFECTIVE DATE:  The first day of the month following thirty-one (31) days of employment.

COST:  Premiums are deducted from paychecks on a pre-tax basis.  Current premium levels can be seen on the pipeline, or U.S. Oil’s web page.

Open enrollment in the dental plan is offered annually in December for a January 1 effective date.

TERMINATION:  Coverage terminates on the last day of the month in which the Associate works.  Participants are eligible to continue this plan under COBRA.
