Healthy Associates Programs

We recognize that Health Awareness is an important aspect of U.S. Oil associates' lives, and that appropriate incentives may assist associates in reaching and maintaining a healthy lifestyle.

WELLNESS INCENTIVES:  Criteria for the "HealthSmart Wellness Incentives" are determined annually and communicated as part of the annual benefits update.  Criteria may vary from year to year.

 

This is a voluntary program.  Associates are not required to participate.  This program is also subject to annual budgetary approval.

 

ELIGIBILITY:  Regular full-time associates and part-time associates with over 5 years of service and 1000 hours worked the previous year.  Participants must be hired prior to the beginning of the program each year and work through the October 31st.   

 

EFFECTIVE DATE:  Immediately.  The program begins January 1st and concludes on October 31st.

 

BENEFIT:  Associates who accumulate 13 to 16 wellness points receive an incentive of $130 for full-time and $65 for qualifying part-time, and accumulate of 17 or more wellness points earns an incentive of $260 for full-time and $130 for qualifying part-time associates.

 

HEALTHSMART REIMBURSEMENTS:  In order to promote wellness among associates, we offer three programs, which enable associates to create a healthy lifestyle for themselves.

 

ELIGIBILITY:  Regular full-time associates and part-time associates with over 5 years of service and 1000 hours worked the previous year. 

 

EFFECTIVE DATE:  After thirty-one (31) days of employment.

 

BENEFIT:  A 50% reimbursement of the first $500 (maximum reimbursement $250) of a qualified program or health equipment purchase (limit one reimbursement of each program available per calendar year) is available.  An associate is eligible for three different reimbursement programs to improve their health.

 

1.   Smoking Cessation Program:  To provide a reimbursement incentive to associates who want to quit smoking in order to create a healthy environment for themselves and others.

2.   Weight Loss Program:  To provide a reimbursement incentive to associates who want to lose weight to maintain a healthy lifestyle.

3.   Health Club or Equipment Program:  To provide a reimbursement incentive to associates who want to use the facilities at their local fitness club or a health equipment purchase, to exercise regularly and achieve maximum energy and health.
