Paid Sick Time is not a right. It is a benefit to provide protection to an associate who is unable to work due to his or her own illness or the illness of a member of his or her immediate family. It may not be used as an extra day off.
U.S. Oil endorses a "Return to Work" policy for both job and non-job related injury or illness. When returning to work following medical treatment for an injury or illness, an associate is responsible for providing his or her supervisor or Human Resource Manager with a Physician's Recommendation Return to Work Form. This form is to be completed by the treating practitioner and is available from your supervisor or Human Resource Manager.
ELIGIBILITY: Full-time associates.
EFFECTIVE DATE: After ninety (90) days of employment for regular full-time non-exempt associates and after thirty-one (31) days for regular full-time exempt associates.
RATE: Sick leave is paid at the associate's regular rate of pay for the hours he or she was scheduled to work.
BENEFIT: After ninety (90) days of employment, regular full-time non-exempt associates will receive one hour of sick leave for each month worked during the first calendar year of employment. Associates hired on or after October 2 will receive two and one-half (2.5) days sick leave after ninety (90) days of employment (which occur in the new year). On the following January 1 the program converts to a calendar year, allowing two and one-half (2.5) days each year.
Associates can use sick time to care for an immediate family member in the event that they are ill and cannot take care of themselves. Immediate family shall mean spouse, child, parent, or sibling.
This policy covers non-occupational injuries and illnesses. Absences due to injuries or illnesses incurred while working for the Company are covered by Workers' Compensation benefits. In cases where Workers' Compensation does not cover time off, sick leave may be applied.
Sick leave does not accumulate. Unused time cannot be carried over from year to year and is not eligible for a cash pay out.
After one (1) week of absence and with proper documentation, the associate becomes eligible for Short-Term Disability.
TERMINATION: Payment for unused sick time will not be made upon termination of employment.
